I am only in-network with the following plans:  Blue Cross Blue Shield (BCBS) PPO, Horizon BCBS of NJ PPO, Aetna PPO, Aetna HMO, Aetna EPO, and Aetna Managed Choice. I do not accept Medicare, Medicaid (e.g., BCBS NJ FamilyCare), or BCBS Managed Care plans (i.e., BCBS HMO, BCBS EPO/OMNIA, BCBS NJ Direct, BCBS Direct Access).

Please note this is an estimate (and not a guarantee) of your health insurance coverage based on what I can see from my end using Availity, which is a web portal that is used by providers to securely access patient information such as eligibility, benefits, claim status, authorizations and other proprietary information.

You are encouraged to confirm the estimate given by me by calling Member Services number on the back of your health insurance card. It is your responsibility to check with your insurance company to confirm Coverage, Benefits, and/or Pre-Certification requirements.
